Probably his height and lantern jaw.
How do they usually dress? What do they wear to sleep? Do they wear jewelry? When traveling with the Doctor, Jack tends to go for a more trousers-and-T-shirt sort of look, much more casually than any other time. Afterwards, since he's more comfortable in uniform, he wears a light-colored button-down shirt with suspenders(braces), tan boots, dark blue trousers, and a wool WWII RAF greatcoat. His leather wristcomp is always on his wrist, even when sleeping, when it usually becomes the only thing he's wearing to sleep in. Aside from the wristcomp, the only thing he wears that even resembles jewelry is his TARDIS key on a chain around his neck, but he later puts it on a keyring instead of wearing it.
Is there anything about their appearance they wish they could change? He's considered plastic surgery to "lift around the eyes, tighten up the jawline." But a bad experience on the Gamestation puts him off the idea, and he's generally content with his appearance.
How would they look as the opposite sex? Prooobably something like this (Yes, that's his actor in drag)
What do they smell like? Why (do they wear the scent or does it occur naturally)? Jack has natural pheromones that make him smell nice to others, like an unusual cologne, so he doesn't tend to wear other scents. I picture it as being slightly minty with flowery overtones.
Do they have an accurate mental picture of their appearance (how they see themselves versus how the world sees them)? Most certainly, and he cultivates his appearance deliberately for maximum effect.
